Challenge

Due to intensive blood loss from hemorrhage, unstable pelvic injuries represent one of the most life-threatening trauma-related lesions with a mortality of 19-31%. A timely control of the bleeding is crucial for the survival of the patient. Current interventions often do not suffice to control massive bleeding or require a highl level of expertise and immediate intervention strategies to control the bleeding include compressing the pelvis with a pelvic binder or inserting a tamponade in the intrapelvic volume. These methods, however, often do not suffice to control the massive bleeding. Surgical methods, on the other hand, require a high level of expertise and an appropriately equipped hospital and thus cannot be performed by first responders.

Innovation

The innovative device is a transurethral catheter with an additional lumen. A balloon can be inserted and inflated inside the bladder. The increase in bladder volume results in a decrease of the intrapelvic volume, which, in turn, can stop the bleeding. The device is ideal as an emergency treatment either for first responders or in the hospital , as urinary catheter placement is a standard procedure in the management of severely injured patients. The procedure is easily combinable with external treatment strategies, such as a pelvic binder.
